Liferaft offers three different levels of Dental Plans in partnership with Ameritas and they are available across the country. These plans provide preventative services such as exams and cleanings and major services such as oral surgery.
All three plans are guaranteed-issue, dependent children are covered until the age of 26, there is no termination age, and are mostly available in 49 states including DC.
The dental policies have three benefit variations which are Maximum Allowable Charge PPO (MAC), Usual & Customary (U&C), and Indemnity. Each plan also includes three Benefit Types, Type 1 Preventative, Type 2 Basic, and Type 3 Major.

The PrimeStar Value Plan only provides Dental Benefits and is best for clients who are on a budget and looking for minimum coverage.
The plan offers three benefit variations (MAC, U&C, and Indemnity) that are dependent on where the client lives. This plan is available in 47 states and DC, the states that are not available for this plan are Maryland, New Mexico, and Washington.

This plan does not offer Childhood Orthodontics, Hearing, or Lasik benefits.

The PrimeStar Access Plan primarily provides Dental Benefits, but also offers limited Lasik Benefits. This plan is the best fit for most clients who are looking for extra coverage but don’t want too high of a monthly premium.
The plan offers three benefit variations (MAC, U&C, and Indemnity) that are dependent on where the client lives. It’s available in 49 states including DC, this plan is not available in the state of Washington. It also offers In-Network and Out-of-Network coverage.
The PrimeStar Value plan offers a Child Orthodontic Benefit. There is no annual deductible, a lifetime maximum of $1,000, and in most states has no waiting period.

The Lasik Benefit offered through this plan has a coinsurance rate of 100% and no annual deductible.

Check your email for a link to the Ameritas appointment form. You must complete this form in order to sell Dental policies—even if you are already appointed to sell other Liferaft products. For agents that sell a dental policy without prior completion of appointment paperwork, Liferaft will not pay any commissions for those policy sales.
